Bulk Secure Forms allow your team to send personalized forms to multiple recipients at once using CSV data. You can pre-fill fields for each recipient, collect signatures or responses, attach supporting documents, and export form results to a CSV file for reporting or processing.
Go to Inbox in the Team Hub and select the Secure Forms tab to begin.
CSV Requirements
To bulk send Secure Forms, you must upload a .CSV file containing recipient information.
Your CSV file:
- Must be under 30 MB
- Must contain Name and Email columns
- Can include additional columns for autofill data
Column headers are not case-sensitive.
Example column headers:
- First Name
- Last Name
- Address
- City
- State
- Zip Code
Each row in the CSV represents one recipient.
Duplicate email addresses are allowed. If duplicate emails are included, forms are still sent individually to each row in the CSV.
If an autofill column is blank, the mapped form field will remain blank for that recipient.
Create a Bulk Secure Form
Click “Create Secure Form”.
You can either:
- Use an existing Secure Form template
- Upload a new PDF form
If uploading a PDF:
- The file must be under 5 MB
Enter a title for the form and choose the folder where the form should be stored.
Then choose the form expiration.
You can set forms to expire after:
- 7 days
- 14 days
- 21 days
- 30 days
- 60 days
- A custom date
After the expiration date passes, recipients can no longer access or complete the form.
Note: If a saved template uses a custom expiration date and that date has already passed when the template is reused, the expiration will automatically reset to 21 days.
Expired Forms
Expired forms cannot be reopened or resent directly.
To resend an expired form:
- Duplicate the form
- Send the duplicated version
Choose Recipient Interaction Type
When bulk sending Secure Forms, you can choose how recipients interact with the document.
Sign and Complete
Recipients are prompted to:
- Fill out fields
- Sign the form
- Upload attachments if enabled
Use this option when recipients need to take action.
View Only
Recipients receive the document as informational content only.
Recipients can:
- Open the document
- Download the document
- View attached files
Recipients are not prompted to:
- Complete fields
- Sign the form
- Submit responses
Use View Only for:
- Tax documents
- Annual statements
- Account summaries
View Only forms can still include attachments and additional files.
Enable CSV Autofill
Select the Autofill from a CSV checkbox.
Upload your CSV file.
After importing the CSV, new autofill field options will appear in the form builder.
Note: Your file MUST contain a column for Name and Email.
Drag and drop autofill fields onto the form where recipient data should appear.
When placing a field, a mapping window will open allowing you to select the CSV column connected to that field.
Repeat this process for all autofill fields you want to use.
Edit Autofill Fields
To edit a mapped Autofill field:
- Click the purple autofill field
- Click the settings icon
You can:
- Change the mapped CSV column
- Duplicate the field
- Delete the field
Add Recipient Fillable Fields
Use the blue field options to add fields recipients will complete after receiving the form.
Available field types include:
- Text boxes
- Checkboxes
- Radio Button
- Signature fields
For each field, you can:
- Mark the field as required
- Edit the field label
- Duplicate the field
- Delete the field
Note: Field labels become column headers in CSV exports. Use clear and descriptive labels for easier reporting.
Pre-Fill Form Fields
You can manually pre-fill fields before sending the form.
Once a field is pre-filled:
- The recipient cannot edit that field
To pre-fill a field:
- Drag the field onto the form
- Double-click inside the field
- Enter text or place a checkbox/signature
Configure Recipient Mapping
Use the Column Mapping section to select which CSV columns should be used for:
- Recipient Name
- Recipient Email
The selected Name column is used in recipient notifications and email greetings.
If the email address matches an existing Customer Portal account:
- The Secure Form will also appear in the customer’s Inbox
Configure Attachments
You can allow recipients to upload attachments when completing the form.
Supported attachment types:
- PNG
- JPEG
- JPG
Each attachment file is limited to 5 MB.
Multiple attachments can be uploaded as long as each individual file remains under the 5 MB limit.
You can configure attachments as:
- Optional
- Required
You can also send additional files with the form.
Additional files can include:
- PNG
- JPEG
Upload these files in the Send Additional Files section.
Save as a Draft or Template
At the top of the form builder, you can choose to:
- Save Draft
- Save as Template
Save Draft
Saves the current form setup so you can finish and send it later.
Save as Template
Creates a reusable version of the form layout.
If editing an existing template, you can:
- Save changes to the current template
- Save as a new template
Editing a template does not affect forms that were previously sent.
Templates can also be shared across users.
Preview and Send the Form
When the form is ready, click Next.
You will be taken to a preview screen where you can review how CSV data appears for individual recipients.
Use the recipient selector dropdown at the top of the page to preview different recipients.
If changes are needed:
- Click Go Back to edit the form
If everything looks correct:
- Click Send Forms
A confirmation message will appear once the forms have been sent.
Click Done to return to the folder where the form is stored.
Recipient Notifications
Recipients receive:
- An email notification when a Secure Form is sent
- An app notification if they are active in the Customer Portal
Recipients will also receive notifications for:
- Reminder sends
- Voided forms
Manage Sent Forms
After sending, the form record displays:
- Total recipients
- Completion progress
- Form status
From the action menu, you can:
- Move the form to another folder
- Archive the form
- Void the form
You can also open the bulk form to view individual recipient statuses.
Send Reminders
Open the bulk form to view all recipients and their statuses.
From this screen, you can:
- Resend notifications
- Send reminders to incomplete recipients
Use the checkboxes beside recipient names to select multiple recipients at once.
Reminder emails resend the same secure form link originally sent to the recipient.
Download Form Data
Once forms have been sent, a Download CSV option becomes available.
The CSV export includes:
- Completed forms
- Incomplete forms
- Form statuses for each recipient
Field labels configured during setup become the CSV column headers.
Completed bulk-sent forms cannot be downloaded individually by internal users.
Recipients can download copies of their completed forms after submission.
Completed forms cannot be edited after submission.
Void a Secure Form
Voiding a form immediately removes recipient access to the form.
After a form is voided:
- Recipients can no longer open or complete the form
- Previously completed forms are no longer accessible to recipients
- Recipients receive a void notification email
When voiding a form, you can enter:
- An internal-only void reason
- A recipient-facing message included in the notification email
After a form is voided, the remaining available actions are:
- Move the form
- Archive the form
Your recipients will receive an email notification letting them know that the form has been voided. They will no longer be able to fill out the form or access their previously completed form.
The status of your form will update to “Voided” and your action options for the form will only be to move the form to another folder or to archive the form.
Secure Form Encryption
Secure Forms are encrypted to help protect recipient information and submitted form data.
Best Practices for Bulk Secure Forms
Test Before Sending
We recommend first sending the form to:
Internal users
- A small customer group
This helps verify:
- CSV mappings
- Field placement
- Text formatting
- Recipient experience
Keep Autofill Text Short
Long text values may overflow outside form fields and become difficult to read.
Use appropriately sized text boxes and keep CSV values concise whenever possible.
Use Clear Field Labels
Field labels become export column headers.
Use labels that clearly describe the field contents.

- Preferred Delivery Date
- Authorized Signature

Notifications for Completed Forms
When a recipient completes a Secure Form:
- The sender receives a completion notification
You can also configure additional email recipients to receive copies of completed forms automatically for a specific template.
To configure notification recipients:
- Go to Inbox > Secure Forms
- Click Create Secure Form
- Locate the template
- Click the three-dot menu
- Select Edit Email for Completed Forms
All listed email addresses receive copies of completed forms for that template.
Customer Notification Preferences
Customers can opt out of Secure Form email notifications from their Notification Settings.
Notification settings are located under:
- Contracts to Sign
If email notifications are disabled:
- Forms still appear in the Customer Portal Inbox for matching portal accounts
Customers can opt out of receiving Secure Forms notifications by toggling off the notification options on the Contracts to Sign category in their Notification Settings.
